When It Comes Back

You recognised it before you admitted it. The sleep first, or the flatness, or the thought you have not had in a year arriving as if it had never left. And underneath the symptoms, a second and worse conclusion: that all of it was for nothing.

Those are two separate things, and only one of them is true. It has come back. It has also come back for a very large number of people who did the work properly: roughly a third within the first year after therapy for depression, and as many as 85% of people who recover from an episode have another at some point.

Recurrence is the expected course of these conditions. It is not a report card on how well you handled the first one.

  1. 1.Two things arrive together: the symptoms, and the conclusion that all of it was wasted. Separate them. Only one is true
  2. 2.Say the fact plainly: this has come back, and it came back before for a lot of people. Around a third within the first year is the ordinary picture
  3. 3.Use the thing that worked last time, even badly, even at a quarter strength. It does not have to be done well to help
  4. 4.Do not start over from nothing. You know the terrain now and that is a genuine advantage you did not have the first time
  5. 5.Getting worse quickly is a reason to contact someone today rather than to wait and see

What is different this time

The first time, none of it had a name and none of it had a shape. This time you know what it is, roughly how it moves and what helped. That is a genuine advantage and it is easy to discount.

You are not starting from zero. You are starting from experienced.

You have a route back

Many services have a faster path for former patients, and almost none of them advertise it. Saying "this is a recurrence" when you call changes how the request is handled.

Say the word recurrence. It triages differently.

You can go earlier

Earlier is the only variable you actually control. Last time you waited until a certain point. The whole opportunity here is to ask before that point.

The advantage is timing, and timing is entirely yours.

On telling people again

The temptation is to hide it, precisely because of how far you had come and how relieved everyone was. Concealment is what turns a recurrence into a crisis, because it removes the people who would otherwise have noticed early.

Tell whoever was most useful last time. They need less explaining than anyone else available to you, and they already know the shape of it.

It came back. So did everything you learned about it, and that part is still yours.
