You are here because it got bad enough to need this. That is not a failure of the last admission and it is not a failure of you. It is the entire purpose of the place, and it worked the way it was supposed to: something got dangerous and you got somewhere safe.
Shame says you failed. The record says you got yourself somewhere safe. Only one of those is a fact.

How to make this admission count for more
Say what did not work last time
The most valuable sentence you can offer, and almost nobody offers it. What happened after the last discharge, where the follow-up broke, what you were left holding.
You are the only person in the building with that information.
Get the follow-up arranged before you leave
With a date rather than a promise, and a named contact rather than a service. The gap after discharge is the highest-risk stretch there is, and it is the thing most worth changing about this stay.
Do not leave without a date. That is the whole ask.
Write down what tipped it
While it is close. The pattern between admissions is more useful than the detail of either one, and it is invisible from inside a single stay.
The gap between admissions is where the information lives.
A second admission is information about the discharge
Not about your character. If the first discharge had held, you would not be here, and something about it did not hold. That is a systems observation and it is worth saying out loud to whoever is planning this one.
Tell one person outside that you are here. Coming back a second time is exactly when people stop telling anybody, and being unknown about is the thing this whole shelf is trying to prevent.
You did not fail the first admission. You outlived it, and then you needed more.
